After downloading one of the files on this page, you can go ahead and fill in the forms using your computer. Please type in the forms as opposed to writing in the information.
The Initial Enrollment Form is needed before any other forms, it is to be submitted with at least a 50% deposit. Once it is received and payment is processed then the spot is saved. This is normally done after a telephone interview with the young person to make certain this is something they want to do and are participating in willingly. But, that is not a requirement.
All payments are non-refundable. But under some circumstances may be transferred to another young person's name, or used as credit for following years.
Also required before June 1st:
- - Copy of Birth Certificate
- - Copy of insurance card (front and back)
- - Copy of Social Security Card (front)
- - Full length photo (front view and left side view) in shorts and T-Shirt
Please read the entire packet as it will answer many of your questions. To save a place in a program the last two pages of the document need to be filled out and send in as soon as possible.
